First make sure you read through the Program Description and Master Agreement. These documents outline the terms and conditions of a Prepaid Tuition Contract.
To qualify, the Purchaser or Beneficiary must be a Nevada resident Or the Purchaser must hold a degree or certificate from a Nevada –domiciled University or Community College.
Purchaser
Person specified who is responsible for payments under the Contract. Must be 18 years of age or older.
When filling out the enrollment form you will need:
- Name
- Address
- Phone
- Email
- SSN/Tax ID
- If listing a Trust as the Purchaser – Trust documents showing Trust name and Trustees
Legal Successor
Person designated by the Purchaser on the Contract to receive information and have full ownership rights in the event of the death or disability of the Purchaser. This is optional but highly recommended.

Beneficiary
Child designated to receive the Educational Benefits of the Program. Must not have completed the 9th grade and not reached the age of 18 at the time of enrollment.

- Birthdate, Age/Grade
Payment at Enrollment
If submitting an online enrollment
- Credit Card or Bank information for $100.00 enrollment fee, optional down payment, or any amounts applying towards a lump sum payment option.
- Bank information if choosing Automatic Withdrawal as your monthly payment method.
- Payroll deduction form to be mailed separately to Program if choosing Payroll Deduction as your monthly payment method.
If submitting a paper enrollment
- Check or Credit card or Bank information for $100.00 enrollment fee, optional down payment, or any amounts applying towards the Contract.
- Automatic Withdrawal form if choosing Automatic Withdrawal as your monthly payment method.
- Payroll Deduction form if choosing Payroll Deduction as your monthly payment method.
