Intent to Enroll

Use this form if you plan on using contract benefits. This form must be signed by both the Purchaser and beneficiary listed on the Prepaid Tuition contract.

Deferment of Benefits

Use this form to request that the Prepaid Tuition Program does not pay out benefits until further notice. An Intent to Enroll form would need to be submitted to resume using benefits. This form must be signed by the Purchaser.

Use this form to request the Prepaid Tuition Program disburse benefits to a private or out-of-state institution that Does Not Bill The Prepaid Tuition Program Directly. Proper documentation must accompany form.
